This doesn't seem to create all the proper folders when I run it. The only thing created are the cfg folder and ui.ini. I wanted to add an hlsl file but even if i create the folder and point mame towards it, it will just crash when i launch a game intil i turn off hlsl support. Am I missing something? Is this expected behavior?

Go to mame dev org and get the full version and just copy past the no nag files into the folder and say yes to overwrite whatever it asks.
